Strengthening Clinical Governance, Confidence, and Emergency Preparedness at HomeCareDirect
At HomeCareDirect, continuous improvement is not something that happens once a year or only when regulations change. It is embedded into how we work every day.
As a nurse-led, clinically governed service supporting individuals with complex needs in their own homes, we understand that excellent care is built not only on compassion and technical skill, but on reflection, scrutiny, and a willingness to ask difficult questions.
One of the most valuable tools in that process is a meaningful audit.
Recently, following the publication of a coroner’s report highlighting concerns around awareness and accessibility of Do Not Attempt Cardiopulmonary Resuscitation (DNACPR) documentation, our Registered Manager undertook an internal review across our service.
The purpose was simple but important:
If an emergency happened today, would every member of our team know where to find DNACPR information, understand what it means, and feel confident in acting appropriately?
It is a question that goes beyond compliance.
It speaks directly to governance, accountability, and ultimately, the safety of the people we support.
Looking Beyond Documentation
The internal audit reviewed multiple areas of practice across our service, including:
Care plan documentation and clinical records
Client home documentation and accessibility of emergency information
How quickly staff could locate DNACPR information in practice
Staff understanding, confidence, and decision-making in emergency scenarios
While documentation was often present, the audit highlighted something important:
Having the right paperwork is only one part of safe practice.
Critical information must also be:
Easy to locate
Clearly communicated
Consistently recorded
Fully understood by every Personal Assistant supporting that individual day to day
This reflects something we place significant emphasis on at HomeCareDirect, personalised care only works when information is not only available, but genuinely understood by the people delivering support.
Identifying Hidden Risks
The audit identified a gap in staff confidence and practical understanding around:
DNACPR processes and legal considerations
Governance responsibilities
Emergency escalation pathways
Clinical decision-making in real-life scenarios
Recognising reversible causes before action is taken
These findings were not viewed as failures.
They were viewed exactly as effective governance should: as opportunities to strengthen practice.
At HomeCareDirect, audits are never about “catching people out.” They are about identifying hidden risks before they become incidents, supporting our teams, and ensuring the individuals we support remain safe in every circumstance.
Turning Audit into Action
In response to the findings, a face-to-face learning session was delivered with clinical staff across the service.
The session focused on practical application, discussion, and reflection, covering:
Roles and responsibilities during emergency situations
How DNACPR decisions should be recorded within care plans
Validity, review dates, and governance requirements
Accessibility of documentation within the client’s home
Understanding reversible causes and applying clinical judgement in practice
Importantly, the session created space for open conversation, allowing staff to ask questions, explore real-life scenarios, and build confidence through discussion rather than passive learning.
This aligns closely with the way HomeCareDirect supports its Personal Assistants and clinical teams across England and Wales.
Training is important.
But training alone is not enough.
The Biggest Lesson
Perhaps the most powerful reflection to come from this audit was this:
We cannot rely solely on training certificates as evidence of understanding.
True competence comes from:
Conversation
Reflection
Practical application
Clinical supervision
Real-world confidence
In personalised care, where Personal Assistants often work independently in clients’ homes, confidence and clarity in emergency situations can make all the difference.
That is why our governance processes extend far beyond mandatory training modules.
Through audits, competency reviews, clinical supervision, reflective learning, and ongoing support, we work to ensure our teams do not simply complete training; they truly understand it.
Building a Culture of Continuous Learning
As we continue developing new collaborative spaces for our Personal Assistants, creating opportunities to connect, share experiences, learn from one another, and hear directly from service leadership, reflections like these become even more valuable.
They remind us that good governance is not about assuming knowledge exists.
It is about checking.
It is about supporting.
And above all, it is about learning together.
